Parent Guardian Waiver

On your way to the Power Play Centre for your Child's Special event, grab their sneakers, stick, helmet, gloves and the parent/guardian waiver you can download here! If you are having a birthday party we would suggest you send it out with the invitations, then all you have to do is be prepared to have FUN! Don't forget to bring your camera!
